Very basic php form needed - URGENT

    Hi,

    I need a 3 field php form needed and the ability to export records to csv.

    That's it. I really want this completed today if possible.

    Should be very easy for an experienced php programmer.

    Some of you may even have a script or software to do this.

    Send me your skype and get this done right now and get paid.

    you can export tables into csv via phpmyadmin. maybe that will help.

    This is for someone that I know and he will get confused with phpmyadmin so I need a real simple export option for him.
