I think it's better to have shorter URLs if at all possible, so taking out of the index.php would be better. I like having just keyword1/keyword2, and not having any excess in there.

Disagree. Your keywords if used should be used in order of relevance. domain.com/keyword1/index.php is far better The structure of a site is FAR more important than spamming the urls. be careful you are not watering down relevancy by your actions.
Old Welsh Guy, What do you mean by the structure of the site itself? You mean overall organization - site map, correct linking of pages to each other? Or keyword density?
