Urgent !! Need a form created ASAP

    Guys i need a programmer to help me out ASAP..

    heres what i need..

    i need a "contact form" type thing... i need to get the input sent to an email and then based on their decision redirect to either a paypal.. buy now button

    hmmm i'll try an explain myself a little better:

    my site is and advertising site for business it has a basic search and a few categories for business to list under.


    i want to give them the option at the end of the form to either:

    request that i send them an invoice

    or

    They can pay online depending on what listing the want standard or featured..

    Paypal buy now buttons..
    -1 year standard -2 years standard -5 years standard
    -1 year featured -2 years featured -5 years featured

    now i want the options above in a dropdown box or check box.. but depending on the users choice, thats where it needs to redirect.. to the correct paypal page... it also needs to send me an email containing the users details from the form: name, business name, address and so on..

    This is and URGENT JOB...

    so on form submit it needs to basicly redirect or if the user chooses "send me an invoice" that they are not directed to paypal but to another page saying thanks for your submission you will receive an invoice in the mail shortly..

    I NEED A TRUST WORTHY PROFESSIONAL I CAN WORK WITH REPLY ASAP IF YOU CAN HELP OUT
