Two listings from one domain in SERPs

Why does Google shows two listings (consecutive & 2nd indented) from the same domain? I don't understand why doesn't it shows the 2nd listing for other domains? Does it pick up the 2nd page arbitrarily or is there any explanation for this?

From How to Interpret your Search Results:

    O. Indented Result
When Google finds multiple results from the same web site, the most relevant result is listed first with the other relevant pages from that same site indented below it.

I know what the O (the Indented result) is.

My question is why doesn't it shows the indented result from other top ranking websites on the same SERP when there are more than one pages (related to the search term) present on "that some other top ranking website(s)"?

    Am I clear enough?

This occurs where two pages from the same site earn a place in the same set of ten results.

So the 2nd indented page may have only ranked 10th, but it is grouped at position 2 (indented) next to the page from the site which ranks first.

To achieve this, the site doesn't need to be #1, it's just any two pages from the same domain that rank in the same set of 10.... grouped together.

... so the reason why most sites don't have a second listing is their second most powerful page (on that search term) is not strong enough to make it into the same set of ten.

If you can achieve top ten for your primary search term, it's a good idea to turn your efforts to putting a second page from your site in the same set of ten so you can dominate more 'screen real estate' in the serps.
