There is nothing you can do to prevent that. Its called "click bombing".

I used to worry about that a lot too, but nothing ever happened. Google will automatically make the clicks invalid. If it happens all too much they may ask you to try to fix the issue but don't lose sleep over it now. I spend months worried about that but it never happened.

You would be banned if the traffic were automated by robots who keep clicking your ads.

You can't be held responsible for the action of others!, unless you are clicking your own ads Google will not ban you.
between, you can't stop fraud clicks neither you have to because, Google already have a structure in place which checks all the clicks against predefined parameters & it accepts or rejects them based on the same.

Depends on what you are promoting and if you are encouraging anything out of the ordinary or against the tos. If you are legit then nothing to worry about. If you are promoting something against the tos or encouraging some iffy content or practices then you never know. Keep it clean and neat and you will be good to go. On a side not that is what I tell the old lady.

Google wont care unless the clicks are coming from your IP... ie: an IP that you logged into one of your own Google accounts. Google keeps track of where you login from and record that IP.
