Here's a project I created a couple of months ago. It started with sending direct mailers out to past residents of a nursing home to check on their current state of health. I used actual photos of the nurses who actually cared for each individual recipient. When the person received the mailer, they were directed to a web landing page (the URL included their name, and the landing page also showed the pic of their personal nurse), which asked them a set of questions on their health, and collected their contact info (phone, email, etc) for future reference.

Here's an ad I did for a local printing/marketing/multimedia company. It ran in a University of Detroit Basketball game program, hence the basketball theme.
Here's part of a direct mail/cross media project, similar to the nursing campaign above, including direct mail pieces that leads the recipient to these personalized websites, as well, as sending out personalized email blasts.

This is a variable-data invitation that was rolled up, tied with a ribbon, then put in a mailing tube and sent out to recipients via US Mail. We wanted to catch the recipient's eye, as they were top-level corporate executives who received numerous invites to all kinds of events. We got rave reviews.
And here's the landing page that the recipients were directed to, to collect their contact info, and register them as attendees:
