It's not very very bad if the content of the every page is different, but it's bad because you're not telling in the title what the page is about. If you repeat the title, you're telling at a start of an index that all the pages are equal. Try to give allways a unique title to each page.
i think you must add more backlinks option to your list . the main criteria for the supp. results is page rank
the most important ones are : different meta descriptions and titles. Also preferably , if you are running a large forum / gallery . review etc website , try to move the title to the beginning , by default many softwares put the sitename up first like : Sitename.com - My first page , Sitename.com - My Two hundred and 14th topic etc . also try using copyscape or similar service from time to time and run a check who is plaigarising you .
Unique Meta Description and Content are the basics , I dont agree with , my new site has 11700 pages indexed with two query strings and I have no pages in supplemental index.
It IS very bad. I have a ASP.NET 2.0 site with masterpages. As anyone who programs in ASP.NET 2.0 knows, you can only have a unique title on your content pages if you have a master page. Keywords and description are taken from the master page. All had unique information and unique titles yet all my pages except my home page ended up in supplmentals. I've added a script that writes a unique description and keywords meta tag to the content and most of them have since moved out of supplementals.
