You will get more number of clicks if your ads are on the following location within a page: 1)Top of page along side header. 2)Right top part(without scrolling). 3)The location where the post content is ending. I am not sure about the image size. Depending upon any of the above locations you have to change the image size.
It completely depends on where you place the ads on your site. If its a blog, then give the large rectangle a try in the sidebars as well as between the Title & the content area. I'm using the Leaderboard ads too.
of course ad size is a fact, but one has to make sure that where the ads are being placed on the website....
never optimize your website/blog with so much add, its look like spammy. Give rich information to your visitor, on that way user will click your add to know more.... Hope this should be helpful
The best ad Placement and ad format always depends on the layout of the website. But I agree that The 300*25 or 336*280 rectangle ads below title works well in most type of websites.
skyscraper and 780*90 work best for me. to improve your earning I think it depends on site layout and ads placement. and traffic ofcourse.
I've always had the best results with a large 160x600 skyscraper to the left of my content. (The typical "navigation" area.)
This is right but I believe that Ads with content will be better option. The reason for this is when there in an image it only gives user one option but the text give 4 to 5 urls.

40% is way too high for Adsense. I heard that people getting more than 15% CTR will be getting manually review by Google.
Your thread title is misleading. I really thought you hit the jackpot Kidding now but here's another good advice: Try screaming ads vs stealth ads. See what pays better for your niche and focus on that. By screaming ads I mean use colors to really attract attention to your ads. By stealth I mean ads that blend with the content. (same color, similar font, etc)
