    I dont think that address and names made u got banned from adsense. I got banned by it two times last year. I didnt know anything abt it then, i clicked on my own ads, and i was banned two times. Even third time i applied for it with my friends name and the same old address, and till now i am with it. U might had linked ur site to any of ur previously banned sites. I heard that, even for this reason they ban users.
