    Accidentally buying nofollow links, not dofollow. Not that this has happened to me, but that would be an ultimate fail really.

    Not having unique meta information for all your pages and building links to them...you can do all the offpage link building you want, but if you don't take the time to get the onpage squared away you are setting yourself up for failure.

    Not having something go anywhere near as planned on Google. Drives me up the wall when standard SEO and backlinking is practiced on every site, but that one or two will just never rank for what's intended on Google, even though all of the others have.

    The 'biggest failure' - and you see this frequently in posts on DP from people who don't understand why they've got no traffic - is optimising for a keyword that has very little search volume.
    Always start with keyword research.

    That would suck considering you're buying the links, but no follow still has value. I have pushed sites to number 1 with only no follow links, granted it wasn't very competitive but it still shows they're worth something.

    The biggest failure would be doing all this off page SEO for a certain URL and that URL doesn't even exist in the first place. FAIL! lol :D

    I had no failures that I made someone decrease rank but like everybody in this world I also made mistakes. My mistakes are usually when I do for many clients and then submit something for one client but register with anothers account :D or when putting "about the autor" that he is selling flowers and article is about computers :p ... so, that's not bad mistakes but just waste of precious time and do something again...

    but my worst mistake was not mistake but just funny thing... It was in my seo beginnings and I scanned some of my opponents... I see where they place links and I do that also... So, I found a place to put a link and did that automatically... Then I saw that my link is places in some "searching for sex" ads... my competitor put that because he probably thought he will increase traffic becaus that niche is so popular... So, about 30 days I had an ad like I am searching for sex with link to my small seo website :p

    There is no question that the biggest SEO failure is to get your website banned by Google (site:site.com 0 results), it happened to me once with on of my test sites that I use to figure out the red zone with Google

    I consider the biggest failure, to be lack of action.
    Far to many people, especially those who are new to the IM game, don't want to take /any/ risk on their own, instead they ask for help at /every/ step of the way. Consequently, they never make any progress.
