Hmm the only way how he could get that is, that he is premium publisher, But Doesn't looks like that site have more than 20.000.000 pageviews per month.

Lol, that's not premium. My dad is IT admin at a health publishing company. He told me about that happening. Google offered them 600x600 block, but since they get their revenue from selling products more than AdSense, which only brings in a few grand a month, they declined.
You "only" need one site reaching the 20.000.000, then you can apply it to your smaller sites as well. I think this site is definitely part of a large network, to they must be premium publisher. Dang. That blending is SICK! The bullets and authority image next to the ad are especially tricky. That's insane. He's gotta be pulling a 60-70% CTR with that above the fold design. I'm jealous.
