tax form in spain !?

I need a tax form in spain (example)

i want know that hoe info the google need for tax in spain !

please give me a screen shot or a .doc file

 

google only needs tax ID or SIN for US publishers. you are a foreign publisher with no US activity (more than likely) therefore google doesn't need your tax info.

 

but i have a tax form in my account

I must fill it?

Yes, you have to fill that form, but as you are not from US, you just need to declare that you are not a US resident or something, and then sign the form at the end. That's it.
