In WordPress, change your comments in Settings -> Discussion to An administrator must always approve the comment

will this also affect other users that are pre approved to leave a comment?

Once we approve is there no going back?

I'll assume you're talking about WordPress.

Have you tried marking their new comments as spam? That might help. You could also place their name, URL, e-mail, or IP in the comment moderation queue or comment blacklist in "Discussion Settings."

If you set it so an admin always has to approve a comment, then I believe it would affect other previously approved commenters.

If I may ask, if you're using Wordpress, why are you having to approve writers in the first place? I turned that feature off because it was irritating. Askimet catches around 99% of the spam that comes in, and you can train it so that it learns how to find other spam.

Is it possible he registered on your blog? Not sure if the do not allow list will block a registered user. Even if it doesn't, that pretty much defeats the whole purpose of a "do not allow" list.

Anyway, try installing this plugin called WP-Ban: http://wordpress.org/extend/plugins/wp-ban/

And place the IP address on the ban list. This should block the IP address from ever being able to visit your site, and therefore can't comment/spam at all.
