snazzy autoresponder sign up with CSS?

    Does anyone know how to make a snazzy autoresponder sign up like this one?
    I'm using mail chimp and there's a css editor. Wondering if there's a way to make it more interesting. The one I have is plain text and colors.

    Could you explain how this is used in your site / when?

    it seems to just be an image in a pdf..?
