I would like that sign up form to be used in a site that looks like this: www. live codes free .com At the top of the page I would like 5 Lines of Text. Each line of text has a green tick as a bullet point then a line of text in size 14. You can put it as "line of text 1, line of text 2" then i willl edit the text later. Then I would like the box with text at the top saying: "For Details on the release of the Card" Then the sign up form. Then the box finishes and I have a few more lines of small text. Very easy work for a good designer/coder.
