Buying Small gift form changes

    I've a small contact form using the php.mailer class. Which is already working. I need some simple changes:

    1- Add a CCV field for credit card info.
    2- Add a confirmation page after and generate an order number. Keep the order number somewhere & do incremental increase.
    3- Send an e-mail to the client with the order confirmation. Right now it only send us the order details info.
    We'd like to do a simple e-mail template to our client in the form:

    Subject: $site.com - Order Confirmation
    Body:
    Thank you for buying your gift certificate with $site.com

    Order Summary:
    Order number: $ordernumber
    Order date: $date

    Shipping information:
    $customer-address

    Total: $order-amount

    I'm paying $5 for the changes, should take less than 10-15 mins of work. Let me know if you are interested.

    As it includes database functionality, i feel price should be at least $10
