Slogan Generator Script

I want a slogan generator script coded in PHP. The script should do the following:

1. Take input from the user using a web form,
2. Pick a random Slogan template from database
3. Replace the variables in the template with the input given by the user.
4. Show the generated slogan.

Requirements:

1. Database driven
2. Ability to add slogan templates in the back end
3. Save visitor suggestions for new slogans
