Simple web forms that don't pull the user away to another site?

I have a client who uses custom forms on her web site to collect user feedback. She's been using formlog.com and is happy with it, but doesn't like that the post-submission landing page takes the user away from her site. Does anyone know of a provider of these forms which are entirely embeddable within her site, won't carry the user away, and are still fully functional?

I realize the free sites are typically ad-supported; that's why I'm willing to pay for this.

Just google phrase like free guestbook script and you will find tons of such scripts.

 

Wouldnt it be easier to either use a free script on your/ her server or even take a single one off hit of paying someone else to write it if what you want is beyond your skill set/ not available for free and thus avoid monthly/ annual fees to avoid ads?

 

Try phpformgenerator very easy to use - you can install it on your site and get an admin control where you can create your forms.
