Simple Script Needed.

    I need a php form created.

    It should function accordingly:

    Authors Name:
    (this should create a folder in the ftp under /uploads/*whatever they type in this text field*)

    Theme File: [Browse]
    This will be where they upload theyre File. (Accepts .p3t files only) and uploads it to the folder they create Above.

    Theme Preview: [Browse]
    accepts jpeg, png, bmp, gif. Then uploads them to the folder they created above.

    Last but not least, image verification.

    Then A submit button.:)
