Buying Simple paypal or google checkout custom form

I currently have a site where people can submit a form which gets sent to my email, they then have to pay seperatly with a paypal or google checkout buy it now button. I am finding it easy to lose track of who has paid.

I need a simple thing where people can fill in my form and then click submit, which then goes to the payment page. Once the payment is done the contents of the form is sent to me like normal.

I need it fairly quickly, I would prefer google checkout.

I know this is quite simple to do so reasonable offers please.
