Simple free picture post and searchable fields

I am looking for a free script, this is a pro-bono project, that will let me add a sub directory/domain to my site. I want it to have maybe 8 fields and a picture.

Those fields would be user definable or look like this:
text, text, text, text, text, numeric, email, link to picture.



I may already have something I can hack, but I thought I would ask for other suggestions

 

try xtrafile , an opensource script ... and configure it for only image upload ....
