Simple Form in Joomla

Hi,
I'd like a simple form that captures the following on a joomla page (under my content):

- Name
- Email
- Phone
- Best Time to Call
- Comments

When submit it clicked it should send the data to my email address and redirect a user to a confirmation page. I'll create the confirmation page and just provide the URL to you.

If you are interested, please let me know how much and how long. I prefer to pay by paypal.

I'd also prefer if you create this independently and then provide me with the code and input files and I'll put them on my joomla site.

you can easily create them yourself by using custom-form components for joomla , just do a google search and you'll get many..
