Buying Simple Email Form Needed

Hey fellas,

I have the html part of the form. I need the php part coded.

Its simple, 9 simple input boxes and 5 upload boxes. I need the input boxes to be emailed to me and the 5 upload boxes to be uploaded to a folder in my server.

After the pics are uploaded, I need the URL to the 5 images send to my email as well in one email.

PM if you can do this. 15+ iTrader required.
