Simple Contact form needed

I would like a simple contact form for my website.

Set out like this:

your email:
confirm your email:

Subject [drop down box]

comments/questions:

After they click the send button I would like a message saying it has been sent to appear on the form. I would also like an automatic email sent to their email address. I would like to be able to edit this email freely.

I would like a spam tester. eg. ask random questions like 2+5, 10-6 etc.

I dont want to use an Iframe.

I would like images to be able to be sent (below a certain file size) eg. browse button so they can send from their computer. Only jpg, gif, bmp etc.

Please PM quotes to me. I would like to choose a programmer this week.
