Make difference between them because all that matters to you is to brief it using different words which is an easy job. Alt text or Title text has different purposes. Alt text is solely designed as an alternative text description for images and to get benefit of those who surf the Web with images turned off. You can use keyword phrase in one or two images on the page, if appropriate.Title text can be used for any page element. They are used to describe links, tables, individual table rows and some other HTML elements.

I wouldn't use to many keywords in the alt-tag, just descripe what is on the picture with your keywords.

I do not think just similarity between image alt tag and title tag would pose a serious threat after all. It may has some exception in case you are using too many of image alt tag with the same keyword repetitive.

I remember reading some information from google that images should be describe properly through alt text when they are needed to be seen in the image search. While title text should be added when you are using an image as your link anchor. This is to give clue to spiders what the link is about since the anchor text is absent.

Employing this is actually good, but overdoing them is something that would raise a red flag on your site. So keep it minimal. Focus rather on getting quality backlinks to your site.
