signing up question

on the publisher sign up form it asks for company name ,
but im just a student

does this mean i don't qualify to be accepted ?

 

If it's required (can't remember) then just put your name.

Ex: John Smith

just one more about the w9 form ...
im in canada
i suppose i don't need since its for U.S affiliates..would they require other info from canadian affiliates to be paid ?
