Sidebar Contact Form for Wordpress Needed

Hello

I need a basic contact form making for a wordpress sidebar - something similar to this example here

I will need the contact details forwarding to two email addresses and would like a logo to be placed at the top of the form.

You can use the cforms plugin and add the contact form in the widgets section.
