i'd go for a). if people search for "George Bush biography", "George Bush Jr biography" or "biography George Bush", they'll get different results. sites optimzed for the exact keywords will show up before those optimised for the phrase then broad. so if you site is well optimised for the exact keywords very well, then i think you'll have a place in the SERPs. about reptition, it shouldn't be a problem in the title title tags. this will optimize for exact. but if it's in the content, then i think it's gonna be a problem. also keep in mind to keep your title short and you'll do good. you may need some keyword research done to sort out your options on the keywords to use.

yes, he's right. google doesn't read the meta keywords. for the words above it's best to use them in your description. optimize for that instead and forget the meta keywords. but also make a place for those keywords in your content. The variant a) is better. It is recommended to repeat your keywords (and if possible use the synonyms of your keyword - google recognize them) in text but no more then 5-10 times (in a 400-800 words article).
We should generally avoid using the keywords repeatedly as it looks artificial and lowers the value of your content. So we should always try to use keywords as less as possible.
Repeating your keywords is absolutely ok, if you do it in a natural way. Having a text without keywords is absolutely not ok -> you have irrelevant text! But you are right..don't repeat them too many times. Simply write your content for users, not for Google, and it will be fine! The best idea probably is to search for some keywords in google, and see, how many times the top sites repeat their keywords. After analyzing 20-30 sites, it is easy to realize what is the truth!
As many of our forum mates are saying that keywords actually are useless if you talk about the google. Google only reads the description tag. I found this personally when i was working for my site, one keyword was not ranking well, so i added some more keywords, but still no change were seen, then i optimized my description tag. I'll advice you to optimize your description tag.
