Setup Database on Godaddy Hosting

I have a site that I need to be able to setup a database on.

For example, I need to have a page where I can go to enter

Name, email, phone, website url for each individual person.

And possibly able to put in a paragraph of text.

Please PM me if anyone can do this!

Here is what I want though.

1) Create Database
2) Create HTML Form so I can just manually go type in name, email, phone, url, handle
3) It automatically creates the url domain.com/folder
4) If you tell me (or teach me how to do this) I will pay $35 for all this.
