Set-up contact form on my website!

I need someone who can set-up contact form on my website. Currently FormMail (cgi) script is installed, but for some reason its not working.

You can have it working, if you can. But I prefer to use PHP script with captcha.

I am not looking for a team of coders, its a simple job and I need a guy with average programming skills to make contact form work.

I do not have any in mind. You can install any generic one, which is available for free, Or see whats wrong with FormMail.
