When you enclose a phrase in "quotes", you are telling Google to search for that exact phrase (rather than just those words sprinkled around the page). Google does not save the enclosing quotes in the database. So you should use the second option in the link (no quotes).
anchor text links should be those keywords that you are trying to rank high for. The addition of quotes will make no difference and should be avoided.

I don't think that's necessary and it looks quite spammy so don't do it. But make sure the exact phrase is repeated 1-2-3 times in your copy. Than build links with anchor text equal and around that phrase.
