Seeking a PHP Form Ninja To Develop Multi-Page Form

    I am working on a website for a health insurance company and have gotten to the point where I need a multi-page (3 steps) form with some built in functionality on each page that will then submit straight through to an assigned email using a nicely formatted HTML email.

    I would prefer the form be done in CSS and cross-browser compatible, but it's not 100% needed.

    The form needs to be done in PHP or some other method, but NOT ASP.

    Please be prepared to show me at least a few forms you have done in the past before contacting me about this.

    I must have this form done before the end of the weekend. It shouldn't be too hard at all for anyone with experience.
