you type in city state zip to supposedly search for a list of local therapists and back comes a page (search results) which is just adsense ads which incidentally are not even city state zip specific. course you don't know until you click

No, I get normal search results. There's also an Adsense ad unit but, it's not related to the search resuls. I think you are wrong.

I tried New York and it gave hundreds of results. I guess, when there are no search results to display, the only thing that is left is an Adsense Ad. unit.

I did click, so I do know - my city returned one result (and that therapist is located in the coty I searched).

Perhaps you choose a city or location that has no listings...

There really is no border on the ads (probably since he/she wants to blend) and it WOULDN'T seem against TOS, but...

If there are NO results, it is violating TOS since the ads are mislabeled.

Super gray for me and hard to answer really... I wouldn't take the risk if the site was mine.

Unless these folks are premium publishers they are violating the TOS by having any ads other than link units on the results page.

Edited: Dug around a little bit more. I don't think these folks are violating TOS because they aren't serving adsense. The search results page is a typical search results page from google, not an adsense search. There are no publisher-ids in the ads on the right on the search results page, either. Therefore I think there is no TOS violation at all.
