Script Needed for CPA Network Postback for Incentive Site

    I am looking to market an incentive offer on a CPA Network. What is the best way to fulfill the visitor's incentive once they complete the incentive offer.

    EX: Azoogle has a CPA Network for creditscore.com that pays $30 and allows incentive traffic. I would like to promote this offer, and send my visitors a free disc in the mail if they complete the creditscore.com offer from my azoogle affiliate link. So visitor goes to my landing page, see my review of creditscore.com, and sees that if they use my affiliate link to purchase a report from creditscore.com, I will send them a CD in the mail.

    So how would I technically set the above scenirio up? If I have 1000 visitors to my landing page, 50 click on the creditscore.com affiliate link, and 20 complete the offer, how do I identify the 20 that completed the offer?

    My understanding is that the best way would be for me to give the CPA Network (azoogle), a "postback" URL. But there has to be a script that works with the postback.

    I'd greatly appriate any help on this, and can reward $50 with PayPal if anyone has the complete script for this:

    Method 1: customer enters in a form on my website with their name and address, and then is forwarded to creditscore.com. Once customer completes creditscore.com offer, CPA network notifies me through "postback" that the customer's subid converted. I then know to ship the customer a CD in the mail. So if 50 people fill out the form, and 20 complete the creditscore.com offer, I should be able to identify the 20 from the 50 that completed the form on my website. I assume there would have to be some sort of script correlation between my initial form (the one that 50 people filled out), and the postback (20 will occur), so that I can recieve an e-mail or download from a database in CSV the 20 of the 50 that need to be shipped.

    Method 2: customer goes straight to creditscore.com offer. If/when customer completes creditscore.com offer, customer is sent to a unique URL back on my website to fill in the form on where to ship their free CD. Here, since only the 20 visitors who completed the creditscore.com offer will be redirected, only 20 will be able to see the form and complete it, so there is no matching involved. Just a redirect after the visitor completed the affiliate URL back to my site.

    Method 3: Almost like a hybrid of the two, this would be kind of like a "CPA Gateway" or "CPA Locker". Customer will be on my landing page, but won't be able to fill out the free cd form until they complete the CPA offer (creditscore.com). The free CD form will be "locked", and will be unlocked on completition of creditscore.com. Basically, the same concept as CPALead where content is "locked", here the free cd form is "locked". The form here would really have to be locked, and could not just be an autotimer like some gateway scripts.

    If anyone can help with the above, I'd greatly appreciate it! I would need this to work with Azoogle (now called Epic), and with CJ.com. So you must have some knowledge experience with these CPA networks and their postback pixels. I can offer $50 with PayPal to anyone that can give me a script for any of the 3 methods above that works with Azoogle and/or CJ.com.

    Does that affiliate network give you the IP of the user who has completed the offer? If so, I can do this pretty easy. PM me
