Review new version PHP Forms site

    Soon, my team is planning to launch a new version PHP Forms site. Online test version of this site - http://phpforms.net/v2/.

    I ask you to look at new design and new site version functionality and tell me my faults. I'd like to start a website soon and without errors :confused:
