I've used Survey Monkey (go to: http://surveymonkey.com/) and the free service is pretty good. However, there are some shortcomings, specifically the gap between what you get for free and what you have to pay for. I did send an email explaining that they're missing a huge market by not offering a pay-as-you-go package, or pay-per-completion deal. Thus far, no reply. Also, they auto-expired my questionnaire and I lost all of my data. I asked them to find my data, they said they'd look into it but did nothing. Other than that little lot (!) the survey tools are pretty good...
I am using PHPSurveyor for 2+ years already. It's open source an boast a ton of great features, including export to Excel/CSV and SPSS script, which is a real time saver! BTW- still need BETA TESTERs ! best,christoph
