Requesting FB page name change, which document is best?

    Hello,

    I'm willing to request my Facebook page name that has 8K likes.. My category is entertainer, I am changing category so that I can request change.

    However, I don't know what document to submit, I mean which one is the best and what else do I need to mention?
    Basically, currently page only has my first name and last is something else, I want to change it to my full name.

    I'd appreciate if anyone can help me out regarding this, pm me if so..

    I'm sorry, I wont mention the name of the page.

    However, if you can help what document would be alright except the utility bill, then it'd be great! :)
