    Try the CPAStorm affiliate network, I know they have many of the "zip code" and "email" submission programs...

    Both NeverBlueAds (3rd link in sig) and AffiliateBot (3rd link in sig) have quite a few email submit campaigns...

    I use Hydra and they are fantastic and they have good support and pay out by paypal. I just did an article on their CPA program here
