Get a DBA (doing business as) name. You can get a DBA from your county. Open a new google a/c including gmail. Give them a new yahoo or other e-mail for you to originally open the account. Don't use old e-mail that is associated with the banning. Also start a blog on their blogspot or blogger a/c. Start blogging - publish original and good quality contents. Also make sure to publish a "privacy policy" as one of the first articles. After publishing about 3 or 4 articles, apply for adsense a/c. Make sure to apply through the blogspot blog. You know how you can add adsense to the blog spot blog, you keep clicking the options until it gives you the option to apply for a new a/c. Choose the business option and use the DBA name. DBA names are considered businesses. Use your name as the contact person. That's all you have to do. As far as I'm concerned IP addresses don't affect this. In a day or so you will get your a/c.
I was thinking if you really earn alot from adsense and they ban your account, we know you can`t make a new account with the same name. If you change your name for real, they won`t know it was you and might work to create a new account. But ofc, this is an extreme solution if you don`t trust your parents/wife (if got any) to create an account in their name.
