You can try politely requesting Adsense to consider unbanning your account. Tell them that you have learned your lesson in the three years that you have been banned. Promise them that you will not defraud them or violate their policies again (And keep your promise).

Bann is permanent but try to contact the support once, might work, 3 years is a long gap.

Bans are permanent. You can try to set up with a different username but I have observed, set up the new adsense account using a different computer than the one that caused you to get banned from in the first place and NEVER access that account from the questionable computer. I know is sounds odd and maybe it is just me. Let me explain my story. I was banned from Adsense due to my own stupidity (family members clicking on the ads using one specific computer) I tried to appeal, no luck. I was able to successfully setup a different adsense account using a family members name. I was able to place the ads on my site and all went well for over a month. I was able to successfully log onto my adsense account on my main computer. One night I logged into my adsense account using the computer in question from the original ban. Within 5 minutes my adsense account was disabled. I made DARN sure nobody I knew clicked on the ads on my site so I know it wasn't abuse. I tested this again using a different account in the same format with the same results. The ban appears to be due to a specific computer. It is a stripped down netbook that doesn't run any other software.

If anyone has any additional insight I would love to hear about it.

Maybe because you have a static IP and you have been in their database starting the day they banned your first account. I don`t see any reason for them to ban accounts just because of the IP class. Maybe my neighbour has an account and he runs a computer from a different IP, but the same class IP as mine. Would he been banned too? It doesn`t make sense. Or maybe they have other ways to catch us, I don`t know.

I was thinking that but it may go deeper, I am thinking at the mac address level. I have several computers that go out of the same router with NAT so they all look as if they are the same IP address. I didn't have the issue with the other computers, only the single netbook.
