I have a similar problem. In my case, the problem is with the host. The host has a limit of 500 emails per domain per hour. My forum keeps hitting the limit frequently. Actually, the number of mails is nowhere near 500, but its a technical glitch. Check your site's mail box for returned mails. You can tell your users to PM you, in case they don't get the verification mail. Something like this. If you are hitting your limit of outgoing emails, try disabling other email features like PM notifications to members, "Report Post" etc.
