yes you can do this a whole heap of ways..

    - You can put a redirect in the code that sends you there. (search google for redirect code)
    - You can make a html link, eg: Please click here to go to here
    - Or you can direct your domain directly to the new one - contact your host or domain provider to do this for you

The best way to redirect a site is to set a permanent redirect (301) to the new site is to set it in your domain control panel itself. for this you need to have DNS hosting (some domain registrars give it free or cheap). or else you can also redirect in your hosting control panel.

but if its a blog in blogger you can just use javascript redirection.. if its a wordpress.com blog then nothing can be done just put a link to your new site.

also a few more points to take care - if your old site is indexed by google you can remove it from the google index by going to google webmaster tools (in Google Webmaster central - just google it :p ) and include your new domain so that google can index your new website and you will not get duplicate content penalty by your old website being already present in the google index.
