Recommend to a friend plugin needed!

    Hello.

    I'm using a WordPress 3.3.1 and I'm searching for a simple plugin, which would allow me to put a "Recommend to Friend(s)" box to any page/post I'd like.

    I need:
    -option to set up to five friends' email boxes
    -chance of writing my own email message recipients will get.

    basically a box with:

    My Name:
    My Email:
    Friends Email:
    Friends Email #2:
    Friends Email #3:
    Friends Email #4:
    Friends Email #5:

    [SEND]

    I think you would need to code such thing, simple plugins already made may touch on this but not have all the features that you want
