What was the reason for you getting banned from adsense? If it's something you think you shouldn't have got banned for then try appealing which, with the volume I guess they receive, it is a very low chance of being accepted back in. That's hard
You should not re-apply on the PC which have been using for the old account, any info should have new changes
its a lifetime ban. you cannot reapply in your own name, address, computer, ip etc anymore. even if you get accepted, you will be banned again before you get paid. your family members or other residents in the house can apply but if google can corelate that account with you somehow, that account will be banned as well

The trouble about trying to open another account in another name is that you would then have to ask that person if they would cash the cheque for you into there bank account.
