Ready-made quiz recommendation?

Hi all,

I'm looking for a program to make quizzes to help customers decide which product they need.

Here's the basic layout of the quiz:

(checkboxes) Please check any problems your cat is having:
Problem 1
Problem 2
Problem 3

(next page only displays the options checked here plus some basic questions)

(radio buttons) How severe is Problem 1?
Mild, Moderate, Severe

(radio buttons) How would you rate the overall health of your pet?
(radio buttons) Over the past month, is their health improving or declining?

Then the form would spit out it's top recommendations based on symptoms and severity, along with a little description for each product.

You can either code it in asp/php/perl/cgi , not that difficult.
