Buying Quick Task - Insert my icontact info into a opt-in form - Need Done Today

    I need somebody who knows html and can add my icontact email information into an opt-in box that I have. The box is designed the way I want it but I don't know how to add my info to it. The guy I got this from said it should only take a minute or two but I just can't seem to figure it out.

    I am paying $5 for this and I have 3 other forms that I might need help with in the next couple of days.
