    Hi guys

    sorry to ask what is a basic question, but im having a few problems with my form to e-mail form.

    The form itself is pretty straight forward and I think the issue lies with the select box, could somebody point out where I have gone wrong?

    The php page is bankclaim.php - which I have included in the quotes.
