Quality Contact Form Generator Needed!

    Hi,

    I am looking for someone to create a Contact Form Generator Script with built in optional captcha.

    Here is an idea of what im looking for: example

    NOTES:
    I would like this to be a 1 page Contact Form Generator.
    I want the captcha to be an optional feature. (make captcha something liek the one seen here).
    After someone builds their form.. the script will display the code(s) to add to their site. (example... mailer.php & formcode.php)


    Budget: $100.00 (payable via paypal)
