These seems to be a phproxy based with some estructure modifications (visuals), you can get it from http://whitefyre.com/poxy/. Also there are other free proxy script but based on cgi, you can get it from http://www.jmarshall.com/tools/cgiproxy/. Both are free scripts. Good luck.
go to source forge there are various ones there that follow the free open source GNU. Php Proxy CGI Proxy come to mind, the former being the popular script and the latter being used for myspace proxy sites.
