Proxy Design and Coding Needed

    Hey guys, I need a proxy designed and coded for $30 or less. I basically just need a nice design for the index.php of the phproxy script. I want the design to replace the green index page and don't need anything custom coded. My budget is very strict and will not go over. Please post here if you are willing to do it... The domain is pr0xysurf.com and the payment can be sent through paypal. I want to go for the web 2.0 look.

    Also,

    The banner needs to say "pr0xysurf.com" with the slogan "rethinking filters".

    I like the design at www.rofflecakes.com, but not the colors.
